Our Basic Affiliate Program consists of the following:

1. $5 Sign Up Fee to cover administrative costs.
2. 20% commissions on sales generated through their Affiliate link (not including applicable sales tax and/or S&H fees).
3. 5% commissions on sales generated through their downline Affiliate's links (not including applicable sales tax and/or S&H fees).
4. 20% discount on personal orders (not including applicable sales tax and/or S&H fees).
5. Commissions paid monthly (minimum commission total of $25 for monthly payments - commissions earned that total less than $25 will be paid when total commission reaches $25).
6. Commissions are paid via Paypal transfer.
7. Affiliates are INDEPENDENT Distributors and are NOT employees of The Chocolate Works & More!!!™ and are subject to responsibility for their own taxes (local, state and federal. personal, business and sales & use tax if applicable), business licenses, insurances, etc.  Affiliates with commissions totalling more than $600 in a calendar year will be issued a 1099.
8. Tax ID number is required. 
9. Affiliates may sell via the method that best suits them. 
10. Affiliates may NOT change the prices of our products at any time.  No affiliate may sell our products for less than what is shown on our website without prior authorization.
11. All Affiliate related orders must go through an Affiliate link in order to earn commissions and/or discounts.  Affiliates will be assigned a log in ID and password for the wholesale section of the website where personal orders (or orders collected through sales other than internet) may be placed.  Orders placed through the wholesale section will NOT appear on the affiliate tracking/information page.  Email orders will not be accepted.
12. No minimum sales requirement to be an Affiliate at this time.
13. Affiliates agree to provide the website where their affiliate link/banner will be placed. 
14. We only accept family friendly websites in our affiliate program and reserve the right to refuse affiliate status if we find the website to be inappropropriate for family viewing.
15. These terms are subject to change at any time and each Affiliate will be notified at least 30 days prior to changes taking effect.

Fill out the form below and click "Submit".  After you click submit you will be automatically issued a username and password. Print this out and keep it handy.  Once your application has been reviewed we will contact you and you will be billed for the $5 sign up fee.  In the event your application is not approved or the sign up fee is not paid within 14 days of the date of signing up the account will be deleted.
